Hydralazine hydrochloride (Apresoline hydrochloride) is a medication used in the management of essential hypertension (high blood pressure). Hydralazine’s mechanism of action in lowering blood pressure is thought to be related to dilating the blood vessels through altering calcium metabolism, resulting in a relaxation effect on vascular smooth muscle. This peripheral vasodilation also increases the output of the heart and may, at times, also increase renal blood flow and blood flow to the brain.

Hydralazine hydrochloride is, by itself, a white to off-white crystalline powder that is soluble in water. After being taken orally, hydralazine reaches peak plasma levels within 2 hours and has a half life of up to 7 hours. After being largely metabolized in the liver, hydralazine is mostly excreted in the urine. When administered with food, hydralazine may reach higher levels in the plasma. Apresoline (hydralazine HCl) is available as hydralazine HCl 10 mg, hydralazine HCl 25 mg, hydralazine HCl 50 mg and hydralazine HCl 100 mg tablets. Hydralazine tablets should not be stored above 30° C. Many regimens start with hydralazine 10 mg four times daily.
